Pharmacist – Specialty Pharmacy & MTM Services provides medication therapy management for patients, including clinical assessments, patient counseling, and drug utilization reviews to optimize therapy.
Responsibilities:
Provides individualized care for specialty patients that include clinical assessments, disease state education, medication therapy management, and benefit investigation.
Performs drug utilization reviews and address medication adherence barriers to improve health outcomes.
Evaluates patient medication records to perform patient outreach and make appropriate therapeutic recommendations to help close care gaps for medication adherence.
Provides medication counseling and disease state education to the patients.
Reviews patient profile to ensure the appropriate use of medications, including drug selection and dosing based on diagnosis, weight/body surface area, allergies, drug interactions, provider notes, and pertinent laboratory information.
Practices pharmacy in compliance with all governing regulatory authorities.
Documents and provides interventions for adverse drug reactions and all relevant interactions with patient and provider.
Promotes the practice of evidence-based medicine by engaging in continue education of new drugs, devices, and medical literature.
Provides drug information and education to patients, caregivers, and providers on therapeutic recommendation and management, including, dosage, drug interactions, side effects, administration, and storage and handling of pharmaceuticals.
Demonstrates the ability to prioritize responsibilities and collaborate with interdisciplinary teams.
Demonstrates competency related to financial assistance programs from federal and state grants, and prescription assistance programs
Understands 340B program, inventory management, and special billing conditions.
Participates in the training and providing education to employees, pharmacy students, and other healthcare professionals.
Provides constructive feedback for improvement and modifications in processes or workflow, but not limited to, scheduling, procedures, and assignments.
Checks all method of communication including email at least once per shift.
Keeps immediate supervisor informed of issues.
Performs other duties as assigned.
Requirements:
Graduate of an accredited college/university of pharmacy (Pharm.D. or B.S. with equivalent experience).
Current Texas Pharmacist licensure in good standing (or eligible).
Certification in Immunization Delivery (or eligible)
Certification in Intravenous Sterile Compounding (or eligible) Computer experience, including Microsoft Windows, Excel, PowerPoint, SharePoint and accounting software.
